We’re Hiring: Dementia Specialist/Care Coordinator

Do you have a heart for walking alongside people living with memory loss, and the family and friends who lovingly provide care for them?

Care Partners is looking for a compassionate Dementia Specialist/Care Coordinator to offer guidance, encouragement, and practical support to individuals and families navigating dementia, mild cognitive impairment, and caregivers.

This part-time, flexible role is 15–20 hours per week and includes care coordination, caregiver support, dementia-informed education, resource navigation, support groups, with some volunteer training, and community outreach.

If you are a good listener, a steady presence, and someone with nursing, social work, health education, gerontology, or related experience who believes families should not have to navigate memory loss alone, we would love to hear from you.
