Our Mission & Vision

Care Partners provides support & companionship on the journey of aging and serious illness.

We envision a community that is fully supported to navigate through our shared journeys of aging, chronic illness, and end-of-life.

Our Values

Client-centered:  We focus on the goals and needs of the whole person.

Respect:  We maintain healthy boundaries & honor the journey of each person we encounter.

Confidentiality: We hold the privacy of client information as paramount.

Collaboration:  We support and work with the continuum of elder services in Cook County.

Our Goals

Enable older adults to stay in their homes and community.

Improve quality of life for people suffering from serious illness.

Support family caregivers and activate compassionate volunteers.

Help people plan and get the healthcare they want.

Educate and advocate on aging, dementia, and end-of-life issues.

Care Partners of Cook County was incorporated as a nonprofit in 2015. It grew from a community effort to provide hospice-like services in 2008 and, in 2010 as a program of the North Shore Health Care Foundation, began to provide hospice-trained volunteers and nurse Care Coordination. Care Partners soon grew to not only serve those with serious illness, but to also support caregivers and to assist at-risk seniors in staying in their homes.

Care Partners Of Cook County is funded by generous donations from the community, the North Shore Health Care Foundation, the Northland Foundation, Cook County Community Fund, Head of the Lakes United Way, Operation Round-up, the Victory Fund, the Medica Foundation, the Lloyd K. Johnson Foundation and through a Live Well at Home Grant from the Minnesota Department of Human Services and federal Older Americans Act grants from the ARDC Arrowhead Area Agency on Aging.
