Care Partners of Cook County was incorporated as a nonprofit in 2015. It grew from a community effort to provide hospice-like services in 2008 and, in 2010 as a program of the North Shore Health Care Foundation (now the Vital North Foundation), began to provide hospice-trained volunteers and nurse Care Coordination. Care Partners soon grew to not only serve those with serious illness, but to also support caregivers and to assist at-risk seniors in staying in their homes.
Care Partners Of Cook County is funded by generous donations from area individuals, businesses, and organizations as well as through federal, state, and local grants.
