At the 2023 ISD 166 Career Fair

Care Partners of Cook County envisions a community that is fully supported to navigate through our shared journeys of aging well and end of life. We collaborate with other aging service providers and across the community to support well-connected, accessible aging services. Care Partners also supports efforts and programs to engage and educate the community to be age-friendly and age-wise.

Ongoing events include a monthly Lunch & Learn series presented with Cook County Higher Education designed to support aging well in our community, and our monthly Memory Café, a safe and welcoming space for people who are living with dementia and those who are caring for them.
