Care Partners is Hiring!

The Care Partners of Cook County Team is looking for an additional Part Time Care Coordinator & Caregiver Consultant!  This is an 8 – 12 hour per week position and has very flexible hours. If you are a compassionate individual who loves working with older adults and their family caregivers – this is a great opportunity for you to join our Care Partners family! This position is ideal for a retired nurse, social worker, or other similar professional who would like to continue to contribute to the community in a meaningful way.  


Responsibilities:
• Provides education, coaching, care coordination, resource assistance and advance care planning to Care Partners palliative care and caregiver clients and their families including intake, assessment, developing goals of care, and follow-up as appropriate.
• Facilitates referrals from other providers, provides program information and referral to prospective clients, and conducts regular outreach through local media and agencies to reach new clients. Works closely with local & regional health care providers to provide continuity of care for clients.
• Maintains accurate, timely, and confidential client records.
• Facilitates the placement and provides oversight for Care Partners companion and respite volunteers and staff.
• With the Director and Medical Director, provides direction and oversight of Care Partners’ client support programs including support groups, respite program and client education programs.
• Works collaboratively with Care Partners staff, volunteers, and stakeholders to develop protocols and training for Care Partners programs and volunteers.
• Maintains professional licensure and mandated education as a nurse, as well as education and training in palliative care, advance care planning and elder care as well as essential resource information for clients.


Qualifications:
• Experience working with older adults and passion for the work.
• Ability to work independently and interdependently.
• Demonstrated excellence in organizational, interpersonal, written and verbal communication skills.
• Articulated support for Care Partners’ mission & vision
• Ability to complete training in Caregiver Consultation, Dementia Capability and other appropriate models of assessment and intervention
• Ability to conduct detailed assessments and develop care plans
• Ability to maintain confidentiality.
• Degree / Education in social work, social services, nursing, health education, and/or related experience
• Possesses a current MN driver’s license and a vehicle for travel to client homes.
