Care Partners was born from a desire to bring professional hospice services to Cook County. Research eventually indicated that this was not financially feasible for our rural area, so in 2010 a dedicated group of volunteers received special training to provide non-medical, end-of-life services for Cook County residents. Care Partners continues to carry a deep commitment to helping our friends and neighbors plan for and navigate their end-of-life journeys with meaning and dignity.
Our specially-trained volunteers offer:
- Friendly visits in the home or by phone
- Delivery, chore, or transportation assistance
- Non-medical respite support
- Caring presence during end-of-life vigil
Our Care Coordinators can:
- Assist with completion of Health Care Directives
- Help you find and connect with local and regional services
- Assist with creation of personal, meaningful, end-of-life observances
- Help you communicate your wishes to friends and family
- Familiarize you and your loved ones with end-of-life progression
- Support family caregivers through a loved one’s death and bereavement
Care Partners also:
- Offers community classes and workshops related to Health Care Directives and end-of-life issues
- Collaborates with area partners such as North Shore Health, Sawtooth Mountain Clinic, Cook County Public Health & Human Services, and the Cook County End of Life Doulas
