Cook County Public Health and Care Partners are thrilled to introduce the Cook County Age-Friendly Initiative, a community-driven effort aimed at enhancing the quality of life for all age demographics within our beloved Cook County. Support and advocacy from our local leaders and community partners have been the bedrock upon which this initiative is built, and together, we are sculpting a future where every resident is valued, supported, and engaged in every life stage.
Key Highlights:
π― Objective: Establish Cook County as an Age-Friendly Community, focusing on vital domains like Housing, Transportation, Health Services, and more.
π Survey: Engage in the Age-Friendly Community Assessment Need Survey to impart your valuable insights.
π£οΈ Forums: Attend community forums, available both in-person and virtually via Zoom, to delve into specific matters.
π Action Plan: An action plan will be developed post-survey and forums, aligning our goals with a vision of a connected, supportive Cook County.
How You Can Participate:
π€ Engage: Take part in the survey and forums to voice your thoughts and experiences.
π’ Spread the Word: Help us reach more community members by sharing information about this initiative.
π Take the online Age Friendly Minnesota Needs Assessment Survey or pick up a paper copy at select locations including Cook County Public Health and Human Services. You can also call Care Partners at 218-387-3788 to take the survey by phone or ask to have a paper copy mailed to you.
π Secure your spot at a Community Forum by registering online or by calling Care Partners at 218-387-3788 to register by phone. Pre-registration is requested, but walk-ins are also welcome.
