Our Services
Most of our services are provided by trained volunteers. Clients are asked to share in the cost to help sustain Care Partners services. Cost shares are voluntary, are based on sliding fee guidelines and help cover costs involved in providing the service.

Senior Rides
Rides to medical appointments in Duluth and wellness activities & errands in Cook County.

Caregiver Support
Coaching and support group to reduce stress, gather support & provide better care.

Chores & Delivery
Help with errands, grocery delivery, and occasional household chores to stay safely at home.

End of Life Care
Death Doulas provide thoughtful planning and presence to support the whole family.

Care Coordination
Nurse support to navigate health issues and dementia, arrange help, and plan for care.

Companionship
Friendly visits and calls for companionship and check-in.

Education & Advocacy
Public programs and trainings on caregiving, dementia, aging well, end-of-life and advance care planning.

Aging Resources
Online and printable Cook County Aging Services Directory and other aging support information.

COVID Community Levels
The CDC’s “COVID-19 Community Level” tool helps communities decide what prevention steps to take based on the latest data. Levels can be low, medium, or high and are determined by looking at hospital beds being used, hospital admissions, and the total number of new COVID-19 cases in an area. Take precautions to protect yourself and others from COVID-19 based on the COVID-19 Community Level in your area.
